On or about September 10th, 2025 Awakenings became aware that an unauthorized party accessed our electronic records system, potentially including private healthcare information (“PHI”). No credit card information was exposed. However certain reports with demographic information were exposed, which included name, age, date of birth, gender, relationship status, employment status, city, and zip code.
We take the privacy and security of our clients very seriously and have taken immediate steps to investigate and remedy this incident to prevent future occurrences. At this time, the incident has been reported to authorities as well as all relevant government agencies in order to protect your data.
Although no credit card information was exposed, name and date of birth can still be used in identity theft or phishing. Please be cautious of unexpected calls, texts, or emails that use your name or claim to be from Awakenings, an insurer, or a government agency; we will not ask you for a password, Social Security number, or credit-card number in connection with this incident. Review your explanation-of-benefits statements and medical bills for services you did not receive. You may also place a free credit freeze or fraud alert with Equifax, Experian, and TransUnion and review your credit reports at AnnualCreditReport.com; additional guidance is at IdentityTheft.gov.
